The consultation checklist is designed to work without an account or a medical-history form.
What is saved
This site stores checklist tick marks in your browser’s local storage. The site code does not send those selections to us or to clinics. Do not use shared-browser storage for sensitive information. You can reset the checklist or clear this website’s browser data.
External services
Clinic links open external websites whose privacy practices apply separately. Map links open Google Maps. Fonts may be loaded from Google Fonts, which receives ordinary network request information. Site hosting may process technical request logs under its own terms. This guide does not include a patient enquiry form or transmit consultation requests.
Optional analytics
Analytics is currently disabled. If optional analytics is enabled in a future release, a consent control will let you choose whether to allow it. Clinic referral measurement is designed to record the clinic identifier, site, language and page type, without sending your search terms, checklist selections or medical information.
Your choices
You can browse the directory without using browser storage. Blocking local storage may prevent your selections from persisting between pages. Print a blank checklist if you prefer to keep your planning offline.
